Hope Wayne Medical Clinic Update
As many of you know, the tragic explosion in December 2010 of Frank's Furniture Store in Wayne occurred in the building directly adjoining the Hope Wayne Medical Clinic. The Clinic building at 35080 Chestnut has major structural damage and is not usable until further notice. We have set up operations in a temporary clinic in the meantime. - Please call the Wayne Clinic at 734-710-6688 for directions to the new clinic site and information about appointments and Walk-In Sessions.
Please pray for the Clinic and especially for the families of those affected by this tragedy.

If you have not been seen at the clinic before and you have an appointment or are coming to one of the walk-in clinics, please bring these completed forms with you.

- Medical records from last 2 year
- Prescription bottles
- Federal tax form 1040 for the previous year
- Medicaid denial letter, if you've applied in the last 2 years
- Social Security benefits letter
- Pension benefits letters
- A $5 donation for service - requested, but not required
