When: Saturday, July 18, 2009, Sign-in: 1:15 - 3:15 p.m. (no appointment needed)
Wear: Shorts and tanks or athletic T-shirts
Bring:

Forms from your school, Head Start, or program
Prior immunization records in any (note: immunizations will NOT be provided)
Parent/Guardian (required)

* $5 donation per physical will be gratefully accepted!

» Necessary lab work will be ordered from SJMHS ONLY.
» This event is for well-child physical exams only. Please call 734-481-0111 to make an appointment to address any illnesses or concerns.
» Like all our services, physicals are for those with no insurance or other means to pay for the services.
